New and Returning Patients

Whether you’re new to Periodontal Associates or a returning patient needing to update your yearly paperwork, you can complete the following patient intake forms before your visit. Please also take the time to review our Privacy Notice.

Please complete the following steps to get started:

1. Call or text us to book your appointment! A front desk associate will be happy to help!

2. Complete your new patient registration prior to your arrival. Patient registration forms should be completed at least 1 day before arriving for your first appointment. You may fill them out electronically, or by print a copy to bring with you to your visit. Patient Registration forms are available in both English or Spanish by clicking the desired button below.

3. If you are a new patient, please bring your insurance card, photo ID, medication list, any referrals or x-rays your dentist has given you, and your completed forms if you opted for a printed copy.

4. If you are a returning patient, please bring any updated insurance cards or medication lists. If you have new insurance, please contact us at least 3 days before your appointment so we can have your estimated copay ready in time for your visit.

In Network Insurance Plans:

We are in-network with the following PPO insurance providers

  • United Concordia

  • Delta Dental

  • UPMC Dental Advantage

While we are not in-network with all insurance plans or companies, you can still utilize your benefits with us!

We will submit all documents and paperwork on your behalf and your re-imbursement check will be sent directly to you.

We offer multiple accommodations for your financial needs so that everyone can receive the necessary dental treatment you deserve! Payment options include:

  • Cash or Check

  • CareCredit*

During regular business hours: please call the location you were seen at to speak with a trained staff member.

After regular business hours: if this is a non-life threatening emergency, call our office to leave a message with the Dr. on call. If this is a life threatening emergency, call 911 or go directly to the nearest emergency room.
